Join our patient participation group using our online form.

We are looking for patients who are willing to give their views about how the practice is doing.

Our aim

• Form a patient participation group and also a ‘virtual’ patient participation group – this is an email community that we can consult on a regular basis about the services of practice.
• We would like our ‘virtual’ group to be representative of our local community and to include patients of all ages and with a wide variety of different needs.

If you would like to be involved we would be most grateful if you could complete our registration form.

How we contact you

• We will contact you by email every now and again, to seek your opinion about services.
• We will only ask questions about the practice in the form of short questionnaires.
• We will add your email address to a contact list.
• Your contact details will be kept safely and securely and will only be used for this purpose.
• If you decide at any point that you no longer wish to be involved, you will just need to inform us and we will be happy to remove your contact details.
